What OS are you using? Can't reproduce that here...

And what should that single item be? The revision graph only shows
targets and sources of copy operations - simple modifications are left
out since that would make the graph way too big. Even with only copies
shown the graph is sometimes too big to handle - just try it once on the
Subversion trunk!
