Seems on the first time, the shell extension isn't loaded fully yet.
You *did* restart after installing TSVN?

And this might help too:

That's expected: there's a checkbox "Save auth data" in the
authentication dialog. If it's activated, Subversion will save your
username/password and use it the next time you access the repository.
