I see what you mean. I think what it means is something like:

"Shows all revisions where changes where made to this item (ie the item
referenced by this URL), not just when it was copied to a different
location".

Of course that's much too long, so in English we might summarize as:

"Shows all revisions where changes were made to this item"

or a bit longer ...

"Shows all revisions where changes were made to this item, not just when
it was copied/branched"

Any more than this will just have to go in the docs.
