In the case of dpkg: painfully, especially as it isn't really
revision-control-based by default. Some people use a hack whereby all
the patch sets are stored as separate patch files and applied at build
time, but it really is a hack. The Debian source format is being
rewritten to make it easier to do this kind of thing.

Of course the current system gives us an enormous incentive to send
patches upstream. :)
