unknown error on file add

i tried to add a file (the file exists on the wc, and not in the
repository), but svn 14.1 refuses to add it, error:

$ svn status
? ./L.html
? ./L_FYI.html
(svaustin:szhth4) $ svn add L.html
A L.html
(svaustin:szhth4) $ svn ci -m ""
Adding forms/L.html
Transmitting file data .svn: Filesystem has no item
svn: Commit failed (details follow):
svn:
file not found: transaction `5s', path `/trunk/forms/L.html'

i tried db_recover (with running apache, with stopped apache), svn
revert, svn cleanup, svn up, but now?
