Try unchecking a couple of icon overlays, restart and see if the Needs-lock
now does appear. In that case, you have too many overlays active at the
same time to display them all. Make a selection of the ones you are
interested in, and the others will be shown as 'normal' (except for
'unversioned' which falls back to 'no overlay')

Report back if the 'needs-lock' overlay still does not appear when it is
the only overlay checked.
