What entries are shown for files and which ones for folders?
Check the registry entry
HKLM\Software\Classes\*\shellex\ContextMenuHandlers\TortoiseSVN

it should exist. If it doesn't, maybe a virus scanner or some other app
prevents the TSVN installer from putting it there.
