Do you have a lot of large files in your project or even just a very large number of files? I have the same issue, but I do keep several working copies of a large project. The TSVNCache program can take it's time to updatre all of these locations. You can improve the speed by targeting the directories that it looks at. Have a look in context->TortoiseSVN->Settings. Go to the Icon Overlays tab. Exclude everything and then include you working copies only. I have a 400Gb disk with lots of stuff that TSVn isn't interested in, so I keep it out of the scan.
