The "save" button is disabled only if TMerge doesn't have a path to save
the file to. Which in case of applying a patch shouldn't happen at all.

The "mark as resolved" button is disabled if the file is marked as
readonly or if TMerge doesn't have a 'save' path.

You could try clearing all settings by removing all registry keys under
HKCU\Software\TortoiseSVN
and
HKCU\Software\TortoiseMerge

that will make all settings/options you've made revert back to the defaults.
