What do you consider as "explorer default settings"? The extension is
not shown by default (at least not on Vista). And I think it's more
important for the TSVN dialogs to show the status columns than the
extension one.
Also: once you've enabled the extension (or any other) column for a
specific dialog (add/revert/...) that setting will be remembered the
next time you show the same dialog.
