Honestly, I don't know.
Somehow holding down the shift key in the explorer context menu always
brings up the TSVN dialogs behind the explorer window.
The strange thing is though that TSVN doesn't do anything different than
when the shift key is not pressed (besides showing more menu entries).

If you ever find out a way to fix this, please tell me.

That post isn't about the explorer context menu but the context menu in
e.g. the commit dialog of the CfM dialog.

For creating a patch from the explorer menu, there's a button "options"
in the dialog that shows up.
