I agree that it's difficult to come up with good succinct descriptions. I
spent a lot of time trying to think of something. But we all have 30 inch
monitors these days...

No, on selecting "Revert to this revision" or "Revert changes from this
revision" there's a message dialog which explains what is going to happen
with a cancel option. In that respect the menu items should probably have
a trailing "..." to indicate that a dialog will be opened - in this case,
an explanatory message box.

On the same lines perhaps "Update item to revision" could have a message
box explaining the action as well (and trailing ellipsis).
