It's relatively easy:
1) Check out http://tortoisesvn.tigris.org/svn/tortoisesvn/branches/1.5.x/Languages into a new WC
2) Copy your portuguese translation into this WC and open it in poedit
3) Update the translation from Tortoise.pot in the branches/1.5.x WC
4) Review all the fuzzy translations & save your work
5) TortoiseSVN->Add Tortoise_pt_PT.po and TortoiseSVN->Commit it

Now your reviewed translation is contained in the 1.5.x branch. I will then take care of including it into the build process.
