Since I only use Vista (for now) if someone reports a crash or something
really wacky which I can't reproduce on XP, I can't really help here.
I've never seen the overlays disappear. But if the TSVNCache.exe process
doesn't run, then of course the overlays won't show up.

I can give you some hints about what you could try:
* in the settings dialog, Icon Overlays page, change the "Status Cache"
to "Shell". If the overlays appear, that means the shell extension
handler is installed correctly. Then change it back to "Default"
(default means it's using the TSVNCache.exe process to get the status
information for the overlays).
* if that doesn't help, uninstall TSVN. Then run the "CleanInstall"
utility you can get from this page: http://tortoisesvn.net/node/191 -
after that install TSVN again. (After uninstalling TSVN, make sure that
all entries in the "Software" panel are really gone. If not, uninstall
the remaining entries of TSVN too).
