This is hard to reproduce just from your description (you already
figured that it would be hard to follow :)).

Can you please either:
* send me your repository (if it's not too big and contains no
confidential data)
* or at least send me the complete log:
   - show the log for the repository root
   - select all revisions in the log dialog
   - hit Ctrl-C to copy all the log data to the clipboard
   - open a text editor (e.g. notepad)
   - paste (Ctrl-V) the log data from the clipboard
   - save the file
   - send me the file

don't forget to tell me for which url you wanted to see the revision graph
