Didn't anyone read the reply I posted to this issue? It's not a bug
perse.

It happens when the case of the filename is changed. You can reproduce
this situation quite easily. Just change the case of one of the
characters from a file in your working copy, e.g. change "readme.txt" to
"readme.Txt" (notice the "T"). The overlay icon will not change, but the
TSVN context menu will no longer recognize the file as being part of the
repository, so the Diff command will not be shown.
