As always, over enthusiastic attempt to be helpful.
Simon says, the method I suggested doesn't work for folders which I
tested and have to agree.
Strangely I got two entries for the added file in the added directory
which was a wrong result too.

Then of course one cannot apply the patch because the new file revision
it's trying to fetch is 0 which doesn't exist to apply the patch to.
So all round discard my answer completely.

Next time I see a suggestion that supposedly overcomes one of my gripes
I will try it before passing on to others.
