I like simple solutions too, but I have tried to do team-wide issue/bug
tracking in both ways (SVN-merged text file and proper web-based issue
tracker) and found that the proper issue tracker was better.

Being able to keep track of the discussion on items is a great help, and
though you can do this with a text file/version control, it's not so easy.

The main problem is the tendency for these things to end-up like Bugzilla
with a billion arcane fields which no-one can be bothered to complete properly.

Let's see what this one's like...
