[TSVN] Glitch when ignoring files from commit dialog

This is a very minor ui bug:

When I select 'SVN Commit' the dialog that appears includes a list of all
the files with their status. If I then use the right mouse menu to ignore a
file the file I ignore disappears from the list and the directory
containing the file is added to the list. So far so good.

I then ignore another file in the same way then when that file disappears
from the list the directory containing it is added to the list as modified
a second time. After a bunch of ignores you get the directory containing
them listed once for each file you ignored.

F5 will, of course refresh the file status box and remove the duplicates.
