You have to delete one of TortoiseCVSes entries at:

HKLM\SOFTWARE\Microsoft\Windows\CurrentVersion\Explorer\ShellIconOverlayIdentifiers

which for some unknown reason still are higher precedenced that the
tsvn entries . . .

(At least so I think).
