The overlays can be as big as the original icon. They just shouldn't be
too big so that the original icon isn't visible anymore. Or (on XP at
least) they then should be made half transparent so the original shines

I know of only one single traffic sign which has a triangle upside down
- and that's not something to tell "attention, dangerous". So that means
  that (most) people won't notice that they have to pay attention to
that file.

Wouldn't a circle be 'too close' to the normal overlay icons? I always
thought (apart from the different color) that the conflicting overlay
should have a different shape so it sticks out. But that's just my
opinion...

Or just add a black line between the two colors.

What color would that 'dark' be then?
