The best way is when it can be reproduced. For anything else, there
is nothing specific I can think of. If there are exceptions or other
info it cannot hurt to include it, even if it does not turn out to be
useful.

Even if a problem cannot be reproduced, some kind of explanation of
what was going on at the time is usually helpful.
