I also would love to see this. :-)

Any Docbook gurus around here care to offer some advice (or some
patches)? That said, we can't do anything too funky or fancy as we've
got to follow O'Reilly's style guide for the most part.

Anything we could handle in XSLT or FOP (that is, outside of changing
the XML) would be super groovy.
