It may be the old way, but it's more functional in some ways and
probably gives better layout. (It does use TeX, after all.)

http://xml.apache.org/fop/extensions.html mentions how to get
bookmarks, but doesn't explain how to get there from a
document... someone will need to take some time fiddling with it.

Maybe I'll take some time to try to do it with passivetex. 8^)
