I never chased it down but I'm pretty sure Visual Studio does this. It
wasn't something happening consistently and of course I was always deep in
getting the coding done so I wasn't able to reproduce it.

But it's possible another developer was using some other tool to touch
files.

There's a contributed hook script to catch when this happens, and I have it
installed on my own server to block commits of files differing only in name
case.
