httpd is running as www-data, and yes it has permissions to access the
password file.
The apache error log is what tells me that it could not open the password
file.

Couple of quick questions:

As which user, is the "httpd" started ?

Does this user(s) has enough permissions to access the repo ?

You can also check apache's access and error logs.
