Adding Revision Number to Changes file comment

When I make a change to some code I also make a note in the Changes
file. I'd like to included with this note the revision number. Of
course, I don't know the number yet because the change hasn't been
checked in yet.

That is, I'd like to be able to type:

    Wed Feb 7 17:50:46 UTC 2007
    Patch provided by foo to fix nasty bug. Fixed in rev $Rev.

and then at checkin have $Rev set to the revision number so after that
point the revision number is plain text and no longer a variable.
