While I have no idea whether it's in any state to use, I've been meaning
to look at Airbag (http://code.google.com/p/airbag/), which is something
that people at Mozilla (and Google? I'm not sure) are developing to use
in Mozilla products as a replacement for Talkback, the commercial crash
reporter they currently use.

I know that Talkback (and so presumably Airbag) also include a
server-side component. We might not be interested in that at the
moment, but I think that if we're interested in crash reporting, Airbag
might still be worth a look.
