What ssh server do you use? The one that comes with Mac OS X 10.4.2
or another one?

On my machine, with the one that comes with Mac OS X 10.4.2, there
is a 10-second delay between "debug1: SSH2_MSG_KEXINIT sent" and
"debug1: SSH2_MSG_KEXINIT received" at connection time. There is
no such delay with the ssh server from darwinports.

Unfortunately the startup time is always a bit slow, but this is
not specific to OS X. And fsh is broken, at least on Debian:
