It is not. It can be, but is not always, nor by default, case-sensitive.

In 10.3 client, there is one supported case-sensitive file system - UFS.

In 10.4 client, there are two supported case-sensitive file systems -
UFS and HFS Case Sensitive.

The case insensitive file system is still the recommended and default
install. (In fact the installer's Erase and Install mode doesn't
offer HFS Case Sensitive as an option.)
