I have to admit that this was my reaction, too. Let me just add one
suggestion that I don't think anyone has mentioned yet:

Get a verbose log of the file in question since before the first
relevant commit. Whenever I've seen problems like this in the past,
I've often been able to diagnose it via the log. One of the more common
causes is that one person either did a commit that failed, and didn't
notice, or did a commit of a bunch of files and mistakenly thought that
the file in question was part of that commit. Either of these are
likely if the first expected commit is missing from the log.
