Take a look at TortoiseSVN and WebSVN, they already provide one way
(reference from log-message to issue id) of integration. Provided I'll
find time, I'll write a hook script, that adds a bugnote to our Mantis
issue tracker in which commit an issue was fixed.

On the other hand take a look at trac, which can already handle
everything you want, but doesn't have a 'real' database behind it. Take
your pick.
