Transactions missing? Report request failed errors?

We are regularly receiving errors, from TortoiseSVN, in the form of:
 
    REPORT request failed on '/svn/repos/!svn/vcc/default'
    File not found: transaction 'oe', path
'/trunk/foo/bar/baz/filename.py'
 
I don't understand exactly what ther error is, nor why we get it so
frequently. Most recently it was having this error on a file which
hasn't been touched in about two hundred and fifty revisions.
 
Reverting, cleaning up, nothing seems to solve it besides deleting
entire working directories and re-checking out, which takes quite a bit
of time... and is very frustrating.
 
What does that mean and is there a graceful way to solve it? I don't
think its TortoiseSVN specific, because once it happens the 'svn'
commandline tool seems to have the same problems..
