I see. When it asks for your user name, be sure to answer with the
fully-qualified username, including your domain prefix:

YARMOUTH\simon.platten

This should prevent it from assuming the hostname is the domain name.

Yes I restarted apached.

I know what you mean about the domain, since it attempts to login with:

EPSGY\simon.platten

However the name of our domain is YARMOUTH and the name of the host
computer (primary domain controller) is EPSGY
