When you uninstall TortoiseSVN, are there any files left in the
installation folder? Try to uninstall reboot and remove these files.

Do you have any other Tortoise* clients or other tools installed that use
the windows icon overlays? There are only sixteen or so overlay slots
available. Windows uses some of them, dropbox and other tools als well.
It's first come, first serve, so there may be none left for TortoiseSVN.
