Thanks for that link - I was aware of the 15 limit, but some more interesting reading.

However:

1. HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\
 CurrentVersion\Explorer\ShellIconOverlayIdentifiers
is the same for both users.
Why do the overlays display for one user and not the other?

2. Yes, I do have DropBox installed, which occupies the first 8 slots, followed by:
01UnsuppModule
02SyncingModule
03SyncedModule
but I do not know where they came from. Then there are the 9 TSVN ones, and 4 more at the end.
As an experiment I renamed 1TortoiseNormal to put 2 blanks at the start, but it made no difference (after a reboot).

3. Why is TSVNcache.exe running under the wrong user? This must be a significant clue, but I have no idea why!
