I'm using TortoiseSVN 1.6.6, Build 17493 and I have the same problem. Simon's explanation is correct, but its use is confusing.

I have a working copy with two committed revisions. Only the "diff with previous version" appears in either the TortoiseSVN or regular context menus if the file is unmodified. Here's a summary of behavior:

If the file is _unmodified_:
* The "diff" entry does not appear.
* The "diff with previous version" entry compares the working copy with the HEAD revision in the repository.

If the file is _modified_:
* The "diff" entry compares the working copy with the HEAD revision in the repository.
* The "diff with previous version" entry compares the working copy to the PREV revision in the repository.

So the "diff with previous version" changes behavior depending on whether the file is modified versus when it is unmodified.
