I had a similar problem a while ago.
The problem was that caches which were already stale/buggy would not be
updated/fixed by newer TSVN versions due to some wrong meta information
which made TSVN think it had already cached these bits.

Shift refresh bypasses the cache. I'm not sure if it inserts the missing
data though. This could probably be answered by Stefan^2.
