On Vista, Ctrl+Shift+right click is all that's necessary, and it does
indeed show Exit Explorer. Unfortunately, this graceful exit of
explorer.exe doesn't do anything to help with the icon loss -- that's an
internal state issue with Explorer, along with various poorly written
programs. So this doesn't help anything, as far as I can see.
