I'm running Windows 7 x64 and having trouble with the overlay icons as well.

My symptoms are slightly different. Here's what I've gathered:
- In the folder (left) pane, Ignored and Modified will show up.
- In the file (right) pane, only Ignored will show up.

At first I had both 32-bits and 64-bits versions of TSVN installed. I tried removing both and installing only the 64-bits version, but the result is the same.

When I had the 32-bits version installed, all icons would show up in the file pane (e.g. in the Save dialog) in 32-bits applications. I don't remember which icons were showing up in the folder pane of 32-bits programs.

Just for the record, this is a clean install of Windows 7, and I'm not running TortoiseCVS. I checked the registry and all TSVN overlay entries seem to be there.
