There have been several TSVNCache bugs discussed here and fixed in the latest nightly builds, ready for the next formal release of TortoiseSVN. I suggest you either

(a) Set TortoiseSVN > Settings - Icon Overlays - Status cache to "Shell" so as not to run TSVNCache at all. Read the help pages for details on what this does.

(b) Try a nightly build from http://nightlybuilds.tortoisesvn.net/1.6.x/ where all the cache performance issues so far identified have been fixed.

If the latter does not help, please report again to this list so that the bug can be investigated.
