protected mode integration

The 32-bit shell extension causes the Tortoise SVN Cache to execute. This is a problem when browsing for files from Internet Explorer. The cache runs outside of protected mode causing a constant prompt with each execution of the extension as it queries status of files.

I don't know the details of how the cache works, but I suspect that it does indeed need to run outside protected mode in these situations. As a result, I think the approach is that an elevation policy key should be created for the cache to always elevate.
