I've experienced the same issue with TortoiseSVN 1.6.1, Build 16129 on Vista.

Deinstalled, rebooted, reinstalled, rebooted.
icons now fine until first checkin, then blank again.

DEL %userprofile%\AppData\Local\IconCache.db didn't help.
