File Metadata: User Interface

To help record information about activities relative to each project
file, I would like to have a clickable form for a user to input data
about the current activities on a working copy.

When a user commits a file, I would like a form to pop up requiring
the user to click on fields to describe their activities and a text
region to input further information. The form would be required to be
updated to allow the commit to complete. The form data should be
collected for subsequent retreival for record, progress reporting, and
statistical analysis. The resultant data should stored along with the
file version and should be collectable at the server once the file is
commited.

I would appreciate all suggestions on how to accomplish these goals.
