Can't reinstall on Vista

I was running 1.5.0 just fine up until today when I decided to upgrade
to 1.5.2. I did NOT uninstall before I installed the new version -
because I don't think I've ever done that before.

After the install, it acts like Tortoise isn't installed at all. The
Cache process isn't running, and no overlays or context menus exist.
The product IS installed, in the proper folder, and short cuts exist
in the start menu. I also can't see any obvious errors in the event
log.

I've tried several uninstall-reboot-install-reboot cycles now, with
1.5.2, 1.5.1, AND 1.5.0, and nothing is working.

What do I need to blow away / fix to get this to reinstall properly?
