Revision numbers are unique. But that does not mean there are 2
revisions 38. What the graph shows is that tags/AD was created in r38
and was copied from trunk_at_36. The very next thing you did was to
create branches/AD-SCR089-SCR-094 from trunk_at_HEAD and the head
revision of the repository comes from your previous commit which
created tags/AD.

I don't know why r41 is not shown - have a look at the revision log
for the repository root. That will show what you did in r41 and may
explain why it is not shown. If it is not clear, then send a
screenshot of that revision in the log dialog.
