some of my files didn't get commited

I'm not sure what version I was on when this happened(1.4.6 or 1.4.7), but I have a project that some key files weren't committed.
Going through the log I can see that the main file in question "project\output\softstart.hex" was added to the repository,
but with subsequent commits it was not committed as well as a few other files.

I couldn't find mention of this in the change log, has anyone seen anything like this?
You can see between versions 235 and 236 that the output directory is not being committed, as if it doesn't think there are any changes.
This continues for the next three commits, where I created a tag based on rev 283. But the hex file is clear back at rev 235.
