Sporadic crashes of the shell extension

Hello all,
I have sporadic crashes of the Tortoise shellextension.
I'm using Vista x64 with Tortoise 1.4.7. Other non-Microsoft shell
extensions I know about are 7-zip 4.57 and UltraISO 8.6.

Explorer crashes into msvcrt80.dll with "The application requested the
runtime to end it in unusual way" or something similar. Unfortunatelly for
me I didn't have symbols downloaded so most of the stack was empty. Most
unfortunate of all, my Visual Studio died while retrieving the symbols
 from Microsoft and something in Vista forcibly killed Explorer.exe I was
trying to debug. All I can remember is that in the bottom of the stack was
a function with a name like ShStartWorkItem or something like that.

I still don't have a reliable way to reproduce the crash.
  * The first time it crashed was when I launched couple of Windows
Explorer windows quickly
  * The second time was when I launched Skype.exe.
Note that even I started Skype.exe, actually Explorer.exe crashed!

So in short, what can I do to pinpoint the bug better? Is there any place
to download pdb files for Tortoise? This at least will give you a good
stack trace. Also, if you tell me where Windows Error Reporting service is
storing its mini-dumps, I can mail the tortoise one to you. If you have
any other tips how to solve this, please tell me.
