There are a few things you could check:
* set the registry entry HKCU\Software\\TortoiseSVN\\OwnerdrawnMenus to
   0, 1 or 2 (1 is the default) and see if it makes any difference
* check what other shell extensions you have installed. Since you're the
   first one reporting such a problem, I suspect some problems when
   interacting with other extensions.
