Apply patch doesn't seem to do anything

I tried applying a patch in unified diff format, but tortoise merge
won't let me save the file, and so when I close tortoise merge, the
files are still unchanged. How should I apply my unified diffs? Or is
this a bug? I think it used to work, but after upgrading to 1.4.4, if I
use TortoiseSVN->Patch on the diff/patch file, it does nothing.
